SMACCA Tool Reimbursement ProgramEach sheet metal worker of member companies is eligible for tool insurance. The worker must fill out carbonless forms available from Local 18 or the Association office. These are 3-part, carbonless forms so are not available for download from our site. There are separate forms for use by Commercial, Residential and Industrial workers.
Each Form must be approved and signed by:
- the owner of the tools
- by a company foreman
by an officer of the company (this confirms that the tool inventory has been inspected and verified)
The inventory must indicate what identifying symbol (i.e., name, initials, etc.) is etched or otherwise permanently marked on each tool.
The inventory form must be on file at the Association office.
If a claim for tool loss must be submitted, please call the Association office at (414)-543-7622 for a Tool Loss Claim Supporting Statement form.
